Summary of Job Function: Provides support, referral, linkage and care navigation to the behavioral health consumer in the outpatient setting. Tasks include assessment of concrete resource needs, monitoring, care planning, and advocacy. Case manager will provide client-centered recovery management for a large volume population. Case manager will address patient adherence, process letters, relay patient requests to the clinical team to address needs. Case manager assists in patient level data management for reporting purposes, and participates in all program improvement initiatives. Must have excellent organization and communication skills, be motivated to work in a fast paced environment and knowledgeable of community resources and benefits.
Job Requirements: Bachelor's Degree required. 1+ year experience in a mental health or social services setting preferred. Must possess and maintain a valid N.J. State Driver's License. BLS required within three (3) months of hire.
